Requesting Your Records
Clients have the right to request copies of their clinical records in accordance with Texas law and applicable privacy regulations.
Requests for records must be submitted in writing. You may request your records by contacting our office using the information below. Please include:
Your full name
Date of birth
Dates of service (if known)
Where you would like the records sent
Records will be provided within the timeframe required by Texas law. Reasonable administrative or copying fees may apply where permitted.

Filing a Complaint With the Licensing Authority
If you have concerns about the professional conduct of a licensed social worker, you have the right to file a complaint with the licensing authority.
Complaints regarding Licensed Clinical Social Workers in Texas are handled by the Texas Behavioral Health Executive Council, which oversees the Texas State Board of Social Worker Examiners.
Complaints must generally be submitted using the official complaint form provided by the agency.
